Whilst we are very happy to treat workcover and TAC patients, please note that our clinic policy is to not directly bill any insurers.
What this means is that patients pay for their consultations at the time of appointment and we ensure that receipts are appropriate to receive reimbursement from your insurer.
Before making appointments for TAC and Workcover patients, we require a copy of the GP referral and completion of financial consent forms stating that you understand and agree to our fees policy.
You can print the required forms from the list below and mail them back with the additional requested documents.
